Robert Half Patient Care Coordinator in Los Angeles, California

Description

Job Description: A healthcare provider in Los Angeles is seeking a Patient Care Coordinator with experience in using the Epic system. This role is critical to ensuring that our patients receive the highest level of care while optimizing workflows and administrative tasks. The Patient Care Coordinator will be responsible for a broad range of direct and indirect patient care activities, focused on enhancing patient satisfaction and outcomes.

Responsibilities:

  • Manage patient care pathways using the Epic system, prioritizing patient needs, coordinating services, and monitoring care movements.

  • Coordinate patient care by scheduling appointments, setting up reminders, and ensuring all necessary medical documentation is prepared and available before patient visits.

  • Enter and maintain accurate and up-to-date patient records in the Epic system, including medical history, treatment plans, and consultations with other healthcare providers.

  • Collaborate with a multidisciplinary team to develop, implement, and evaluate individualized patient care plans.

  • Serve as a primary contact for patients, handle patient communication, and resolve and escalate any patient-related issues promptly and professionally.

  • Act as a liaison between patients, healthcare providers, and insurance providers, ensuring seamless communication and delivery of care.

  • Provide education and guidance to patients and families about treatment plans, medical procedures, and healthcare resources.

    Requirements

Qualifications:

  • Minimum of an Associate's degree in healthcare administration, nursing, or a related field.

  • Previous experience in a Patient Care Coordinator role or a similar role.

  • Proficiency in Epic or a similar healthcare software is a must.

  • Excellent organizational skills and an ability to prioritize and manage multiple tasks simultaneously.

  • Outstanding communication and interpersonal skills.

  • Strong understanding of medical terminologies, procedures, and the healthcare industry.

  • Knowledge of medical office management systems and confidentiality obligations.

  • Ability to work independently as well as part of a team.
