Job Title: Case Manager

Location: San Francisco, CA (100% On-site)

Robert Half is seeking a dedicated and compassionate Case Manager for our client in the non-profit industry, based in San Francisco. Our client is committed to providing support and advocacy for the clients they serve and is seeking a service-oriented individual to join their team.

Job Description:

The Case Manager will be responsible for providing supportive and direct case management services to clients, which includes intake assessments, benefits assistance, referral services, and creating service plans.

Responsibilities:

  • Carry out individual client assessments, and create tailored case management plans.

  • Coordinate services and resources for clients in accordance with their specific needs.

  • Monitor and document client progress towards their goals.

  • Assist clients with applications for services, programs, and benefits.

  • Advocate for clients' needs and rights across various settings.

  • Maintain a caseload of clients, providing support and follow-up.

  • Adhere to professional standards and procedures, and practice confidentiality.

  • Collaborate with other staff and community agencies to maximize client support.

Requirements

Preferred Qualifications:

  • Bachelor's Degree in Social Work or a related field.

  • A minimum of 2 years' experience in a case management or social services role.

  • Strong knowledge of community resources and agencies.

  • Excellent communication and interpersonal skills.

  • Solid organizational abilities and capacity to manage multiple case loads effectively.

  • Ability to empathize with and advocate for clients' needs.

  • Proficient in MS Office Suite and case management software.

  • Ability to handle confidential information with the utmost discretion and professionalism.
