Description
We are in search of a Case Manager based in San Francisco, California, 94102, United States. The role involves managing units of formerly homeless single adults, conducting needs assessments, and providing a full range of case management services. This is a contract employment opportunity that offers a chance to be part of a dedicated team.
Responsibilities:
• Oversee a caseload of formerly homeless single adults, many of whom have mental health disabilities, substance abuse issues, and/or medical issues.
• Conduct outreach visits to establish initial contact and maintain engagement with the individuals under your care.
• Carry out initial and ongoing needs assessments, and provide a full range of case management services.
• Aid in maintaining housing through building rapport, frequent engagement, and effective intervention strategies.
• Assist in establishing appropriate benefits and aid in making regular rent payments.
• Handle unit habitability issues and accompany hotel managers on pest control and unit inspection visits.
• Refer employable individuals to appropriate job placement programs as needed.
• Foster an active community and environment of support by hosting regular community events, tenant groups, and social activities.
• Refer individuals to appropriate services based on their needs and support them in successfully connecting with those services.
• Participate in and support engaging individuals in agency-wide initiatives.
• Respond to tenant-related crises, providing de-escalation intervention and support.
• Collaborate with various departments and outside providers to support and advocate for the individuals under your care.
• Maintain current and thorough documentation and database records of tenant contact and services provided.
• Respond to all phone messages, tenant requests, staff requests, and emails in a timely manner.
• Maintain confidentiality of tenant records and concerns.
• Attend all scheduled meetings and participate in meetings as requested.

Requirements
• Possess a minimum of a Bachelor's degree in a related field such as social work, psychology, or nursing
• Proficient in administrative tasks including data entry and filing
• Previous experience in case management or a related role would be advantageous
• Excellent written and verbal communication skills
• Ability to work independently and as part of a team
• Strong organizational skills and the ability to manage multiple cases simultaneously
• Knowledge of relevant laws and regulations pertaining to case management
• Proficiency in using computer software such as Microsoft Office Suite
• Ability to handle sensitive information with discretion and maintain confidentiality
• Excellent problem-solving skills and the ability to make decisions under pressure
• Demonstrated ability to build and maintain relationships with clients and stakeholders.
