Description

We are seeking a Case Manager to be part of our team, offering a long-term contract employment opportunity. The role is based in San Francisco, California, and involves managing and assisting formerly homeless single adults, many of whom have mental health disabilities, substance abuse issues, and/or medical issues. The Case Manager will be responsible for conducting initial and ongoing needs assessments, providing a full range of case management services, and collaborating with various departments to support and advocate for clients.

Responsibilities:

• Effectively manage a caseload of formerly homeless single adults, providing necessary assistance and support

• Conduct initial and ongoing needs assessments, identifying the requirements and goals of the clients

• Regularly conduct outreach visits to clients, and provide support as and when needed

• Implement strategies for housing retention and life quality improvement

• Assist clients in establishing and maintaining appropriate benefits and making regular rent payments

• Address unit habitability issues and accompany hotel managers on monthly pest control and unit inspection visits

• Refer employable clients to appropriate job placement programs

• Foster an active client community by hosting regular community events, tenant groups, and social activities

• Maintain comprehensive and up-to-date documentation and database records of client contact and services provided

• Respond to client-related crises, providing de-escalation intervention and support to highly agitated clients

• Collaborate with various departments and outside providers to support and advocate for clients.

Requirements

• Possess a Bachelor's degree in a relevant field such as Social Work, Psychology, or Healthcare

• Proven experience as a Case Manager or in a similar role within healthcare or social services

• Strong knowledge of case management principles and healthcare management

• Familiarity with professional and technical emerging knowledge

• Proficient with relevant software, including Microsoft Office Suite

• Exceptional communication and negotiation skills

• Ability to build and maintain professional relationships with both clients and healthcare providers

• Strong organizational and time-management skills

• Ability to work independently and collaboratively within a team

• Experience in providing administrative assistance, data entry, and filing

• Ability to maintain confidentiality of sensitive information

• Strong problem-solving abilities and critical thinking skills

• Must be empathetic and supportive with a genuine interest in helping others

• Ability to handle a high-stress environment and make decisions under pressure

• Must possess a valid driver's license and have reliable transportation for client visits.
